This appendix provides a list of items by scale for the full-length Conners 4®, the Conners 4®–Short, and the Conners 4®–ADHD Index. These items and scales are provided to aid use and interpretation of the tool, but the contents of this appendix are copyrighted/trade secret material that are not intended for public disclosure.

Legend for all tables in this appendix: P = Parent; T = Teacher; SR = Self-Report; n/a = not applicable (the item does not appear on this form); (R) = Reverse scored item.


Conners 4 (Full-Length)

For the full-length Conners 4 forms, if the Self-Harm Critical Items and/or the DSM Conduct Disorder Symptoms Scale are disabled prior to administration (see chapter 2, .Administration, for details on disabling sections in the MHS Online Assessment Center+), item numbers in this appendix will be different from what is presented on the assessment. However, the Item Responses section of the digital reports will provide both the item numbers presented to the rater (referred to as Custom Item #) and the corresponding original item number (referred to as Standard Item #).
