The three-phase development process resulted in the final set of items and scales for the Conners 4 Parent, Teacher, and Self-Report, with 114, 106, and 115 total rated items on each form, respectively. Three open-ended questions were added to provide the rater an opportunity to report additional information that may not have been covered by the rating scales. The first question asks the rater to provide extra information on the pervasiveness of the problems reported on the different domains of functioning (i.e., at home, in school, at work, and with peers). The second question asks the rater to report on any other concerns about the youth, and the third question asks the rater to describe the youth’s strengths and skills. Thus, the final forms consist of 117, 109, and 118 total items for the Parent, Teacher, and Self-Report, respectively.
